The patient’s hospice care is coordinated through the patient’s primary physician and hospice medical director.  The scope of hospice services and any medical equipment and medications needed by the patient are provided as ordered by the patient’s primary physician and the hospice medical director.  Our hospice medical director in conjunction with the patient’s primary physician certifies the patient’s eligibility for hospice services.
